This service may be suitable if you:

• Have an existing type 2 diabetes diagnosis and want support as part of your overall heart health
• Would like ongoing review of your risk factors alongside your GP

This isn’t suitable for a new diagnosis, or for type 1 diabetes.

Type 2 diabetes management is assessed individually as part of the broader CVD Risk Reduction Program, and recommendations may change as your risk factors change.

Your pharmacist will assess your individual suitability, including your relevant medical history and current symptoms, before proceeding. 

This service supports ongoing management of an existing type 2 diabetes diagnosis; a new diagnosis or type 1 diabetes requires assessment by your GP. 

Ask your pharmacist for the relevant Consumer Medicines Information (CMI) if a medicine is provided.

Before Your Consultation

Be prepared to discuss:

• Your recent blood sugar readings, if you monitor them
• Your current diabetes medicines, if any
• Relevant family or medical history

During Your Consultation

Your pharmacist will review your risk factors and current management before discussing your ongoing care.

The consultation may also include advice on lifestyle factors and medicine adherence, often alongside your Cholesterol and Hypertension consultations.

Will this consultation involve a blood test?

Not typically — it’s a review of your existing diagnosis and management; formal blood testing is arranged through your GP.

Can this diagnose diabetes for the first time?

No — a new diagnosis needs assessment and testing by your GP.

Does this cover type 1 diabetes?

No — this program is specifically for type 2 diabetes.
